Application Settings

This page can only be accessed by account administrators.

The Application Settings page allows you to view/manage global settings for your Practis Forms account.

Your company name. Displayed in title tags, email notifications, and alert messages.

Website Address

Your company website address.

Terms of Use

Displayed in place of the default Terms of Use. If this area is left blank. Practis Forms will display a default Terms of Use message.

List Forms on "Terms of Use" Page

If selected, all of your Practis Forms will be listed on the Terms of Use page.

Timezone

The timezone used for recording submissions and access log data.

Providers

This is where you will enter your providers. Please enter one per line. Lines will be sorted alphabetically and will be displayed as a drop-down menu in the forms that call for this information.

Locations

This is where you will enter your office locations. Please enter one per line. Lines will be sorted alphabetically and will be displayed as a drop-down menu in the forms that call for this information.

Once you have made the changes you need, please click the “Save Changes” button below to save your changes.

Enhanced Security

Enhanced Security provides additional measures to keep your data secure by tracking users' IP addresses and providing challenge-response questions when users sign in from new IP addresses and when resetting forgotten passwords.
