A patient mentioned that they couldn’t submit our form. What could be the problem?

There are a few requirements for visitors when submitting a form.

  1. Practis Forms requires that visitors user a modern web browser. If a visitor is not using a modern web browser a warning message is displayed prominently at the top of the form. The following web browsers are supported by Practis Forms.
  • Internet Explorer 9+ (recommended version: 11)
  • Firefox
  • Google Chrome
  • Safari
  • Opera

2. Practis Forms requires that visitors enable JavaScript to submit a form. If a visitor does not have JavaScript enabled a warning message is displayed prominently at the top of the form.

  1. If your form includes required fields visitors must fill out all of the required fields in order to submit a form. If a response to a required field is missing a warning message is displayed near the question. Solution: We could remove the requirement on certain fields.
  1. If your form includes formatting validation fields (phone, date, etc.) visitors must format their response in specific way in order to submit the form. If a response is not formatted correctly a warning message is displayed near the question. Solution: We could remove the formatting validation on certain fields.
  1. If your form includes a Security Question to cut down on SPAM submissions, visitors must complete the Security Question in order to submit the form. If the security question is not answered correctly a warning message is displayed near the security question. Solution: We could remove the security question.
