Login / Password Reset

To access Practis Forms, you will need a web browser and an Internet connection. JavaScript and Cookies must be enabled in order to use Practis Forms.

All modern browsers are supported, including:

  • Internet Explorer 9+ (for a better experience, use Internet Explorer 10 or above)

  • Safari

To access the Practis Forms, navigate to the following website in your browser:
https://practisforms.com/login/

The login page will appear.

To login, use your email address and password. You may have already received your password. If not, contact Practis.

* It is recommended that you log in every few days to check for new submissions. Submissions will be automatically deleted after 30 days.

Resetting Your Password

If you forget your password, you can reset it by selecting Forgot your password? from the login page.

The Reset Password page will appear.

Enter your email address and further instructions will be emailed to you. If you don’t receive an email after a few minutes, please check your junk folder.

Logging Out

It is very important that you always log out of your account when you are done. Due to all of the sensitive information that resides in your forms, this is extremely important that you always log out.
